Patient Centered Medical Home - Improving Health Care, One Relationship at a Time

The Family Health Clinic at Malcolm Grow Medical Clinics and Surgery Center (MGMCSC) is making it easier for patients to schedule appointments and discuss medical concerns with their provider through implementation of a new health care delivery model called Patient Centered Medical Home (PCMH). This initiative improves the way health care is organized and delivered.

PCMH encompasses five key attributes, which improve health care through a focus on the patient, continuity, coordination, access and a commitment to quality.

PCMH promotes partnerships between patients and their assigned medical team and excels at building clear and open communication. Patients and families are core members of the care team and are fully informed partners in establishing care plans.
"Patients are empanelled to their own personal provider and their care is managed by the same medical team at every visit. Patients and providers develop a personal relationship, which contributes to better knowledge and understanding of their health care needs and course of treatment. If a patient's assigned provider is not available, they will be seen by a partner on the team, improving overall continuity," said Lt Col Jon Geno, Family Health Flight Commander.

Enhanced coordination focuses on all aspects of the patient's medical needs. The PCM will arrange any care for patients that cannot be provided in house.

"We've revised our appointment process to allow patients to discuss several different problems in one visit to better resolve overlapping health care concerns," said Geno.
Access is improved through a variety of means including opening schedules, adding appointments, telephone communication and secure online messaging with the medical team via MiCare.

"Patients may now make appointments, request medication renewals and receive lab results using MiCare. There is even an option for web visits which make it possible to meet with your provider, virtually (online)," said Geno.

Clinics provide one-stop shopping and make it easier for patients to access the care they need.

"To enhance quality, we focus on what the patient needs and coordinate our provider's time and schedules around that. Our care givers raise the bar in defining high-quality care by emphasizing access, health information technology, and partnerships between clinicians and patients to provide highly coordinated care and long term relationships. It transforms primary care into what patients want it to be," said Geno.
